
RESIDENTAL SERVICES
(Carlson House)
Eggleston’s Residential Services will provide quality living options for adults with Intellectual Disabilities that encourage the individual’s participation in community life and the surrounding area. Key elements to these services include respecting each individual’s uniqueness; providing assistance with the physical, emotional and spiritual needs of each resident; understanding the concerns and needs of each resident; and insuring that all support, skills training, assistance and supervision is tailored to meet the individual’s needs and that all services are provided in a caring and dignified manner.
The Staffing pattern in the group homes will be a twenty-four hour, 7 day a week schedule. The individuals that will be served at the Carlson House will fall into the moderate range of Intellectual Disabilities with some having communication deficits, a need for medication management and health monitoring. Residential Support Services consist of training, assistance and/or specialized supervision provided in a licensed/certified residence considered to be his or her home to enable an individual to acquire, improve, or maintain the health status and functional skills necessary to live in their community. Emphasis will be on individualization by utilizing a person-centered approach that empowers and supports each individual in developing his or her own lifestyle. By assisting people in the development of their skills, abilities and interests through onsite and community-based training’s, the goal of enhancing these individuals’ lives is continually pursued.

Populations served:
Individuals over 18 years of age or older with Intellectual Disabilities that are eligible for MR Medicaid waiver.
Admission criteria:
Must have Intellectual Disabilities
Eligible for Medicaid Waiver, Community Services Board or Private Funding.
Ability to benefit from services, including willingness to participate in community experiences, provided in individual and group settings with the levels of staffing provided under current funding structures.
Medical needs do not necessitate skilled Nursing Care during program hours
Must be 18 years or older
Must be medically stable prior to admission including free from communicable diseases that pose a serious threat as defined by public health authorities (ex. Tuberculosis)
Must not be deemed a threat to self or others (including no recent history of serious aggressive or violent behavior).
Not actively abusing drugs or alcohol.
Must submit required intake paperwork for admission.
Discharge Criteria:
All applicable individual goals have been met.
Acceptance in alternate program.
Violent or disruptive behavior that poses danger to self or others and that has not been successfully controlled with appropriate support methods.
Consistent refusal to participate in programming and goals.
Presents needs, including medical and clinical, which cannot be met with available resources.
Services no longer required, excessive absence from residence or does not benefit from program.

Staff Qualifications:
Trained and holds current CPR/ First Aid certification
Trained and holds current Medication Assistance certification
Trained and holds current Mandt (behavior management) certification
Trained Orientation to M.R. Waiver
Free of communicable disease in any form.
Cleared through criminal background investigation and drug testing.
Has demonstrated skills and abilities in supporting and training people with cognitive/physical disabilities.
Accreditation & Licensure:
Licensed to provide congregate residential support services through the Department of Mental Health, Mental Retardation and Substance Abuse Services.
