OBRA PROGRAM

The OBRA Program provides specialized services to persons with Intellectual Disabilities or related conditions who reside in Medicaid-funded facilities. These services consist of those that persons cannot access through a nursing facility such as training in the areas of communication skills, Independent living skills, Assistive Technology, Environmental modifications, and community access. These services are meant to improve and maintain general health and well being, as well as, maximize self determination and independence and are prioritized based on an individuals most critical specialized needs. Services are available Monday through Friday and individualized schedules are developed based on the needs of each individual. as well as, available resources. Services are offered at the nursing facility or at a community base center depending on their desires, needs and limitations. An individual service plan is developed from standardized assessments, observations, medical records, family and/or associates interests. These individualized services are ultimately designed to enhance the quality of life for those individuals involved in the OBRA program.

  • Populations served:

  • Individuals with Intellectual Disabilities or related conditions who reside in a nursing facility and have been identified by the review team contracted by the state to receive OBRA services. These individuals have been identified by the Funding source as appropriate and eligible for the services.
  • Admission criteria:

  • Eligibility for OBRA services is determined through the DMHMRSAS and the funding source (s). Eligible consumers are those who need and can benefit from specialized services and have a related condition, which manifests before 22 years of age.
  • For Offsite services-medical needs do not necessitate skilled Nursing Care
  • Must be medically stable prior to admission including free from communicable diseases that pose a serious threat as defined by public health authorities (ex. Tuberculosis)
  • Must not be deemed a threat to self or others (including no recent history of serious aggressive or violent behavior).
  • Not actively abusing drugs or alcohol.
  • Must submit required intake paperwork for admission.
  • Discharge Criteria:

  • All applicable individual goals have been met.
  • Acceptance in alternate program.
  • Violent or disruptive behavior that poses danger to self or others and that has not been successfully controlled with appropriate support methods.
  • Consistent refusal to participate in activities including community experiences.
  • Presents needs which cannot be met with available resources.
  • Services no longer required, excessive absenteeism or does not benefit from program.

  • Opportunities for vocational services and wage earning:

  • Off site-Minimal-non integrated work opportunities are limited and are paid on a piece rate basis.
  • Staff Qualifications:

  • Trained and holds current CPR/ First Aid certification
  • Trained and holds current Medication Assistance certification
  • Trained and holds current Mandt (behavior management) certification
  • Free of communicable disease in any form.
  • Cleared through criminal background investigation and drug testing.
  • Has demonstrated skills and abilities in supporting and training people with cognitive/physical disabilities.
  • Accreditation & Licensure:

  • Eggleston Services, Inc. OBRA services are CARF accredited in Personal and Social Support Services and are licensed through the Department of Mental Health, Mental Retardation and Substance Abuse Services.
