Day Support Services
Tanners Creek Horticultural Services-Day Support program is designed for individuals who desire a year round day program based on improving the quality of life for individuals with disabilities through the use of a horticultural environment. Given Tanners Creeks location next to the Zoological Park, associates have the opportunity to visit the grounds to see the animals, as well as, tending several gardens that are frequented by the public.
The hours of program operation are offered from 8:00 am to 1:30 pm, Monday through Friday. All activities provided through Tanners Creek focus on growth in independence and self- actualization. This is achieved by training in the areas of socialization, communication, behavior development, personal care, aerobic exercise and community access. By assisting people in the development of their skills, abilities and interests through onsite and community-based training’s, the goal of enhancing these individuals’ lives is continually pursued.

>Populations served:
Individuals over 16 years of age or older with Intellectual Disabilities or Developmental Disabilities that are eligible for MR Medicaid waiver or DD Medicaid Waiver Day Support services.
Admission Criteria:
Must have a documented disability
Eligible for Medicaid Waiver, Community Services Board or Private Funding.
Ability to benefit from services, including willingness to participate in horticulturally related activities involving plants and soil, provided in group settings with the levels of staffing provided under current funding structures.
Medical needs do not necessitate skilled Nursing Care during program hours
Must be 16 years or older
Must be medically stable prior to admission including free from communicable diseases that pose a serious threat as defined by public health authorities (ex. Tuberculosis). Should not have allergies to plants (or they may be controlled by medication)
Must not be deemed a threat to self or others (including no recent history of serious aggressive or violent behavior).
Not actively abusing drugs or alcohol.
Must submit required intake paperwork for admission.
Discharge Criteria
All applicable individual goals have been met.
Acceptance in alternate program.
Violent or disruptive behavior that poses danger to self or others and that has not been successfully controlled with appropriate support methods.
Consistent refusal to participate in activities including community experience.
Presents needs which cannot be met with available resources.
Services no longer required, excessive absenteeism or does not benefit from program.
Program Capacity:
20 total daily
Opportunities for vocational services and wage earning:
Minimal-Work opportunities are available and are paid on a piece rate basis or hourly- based on productivity. These opportunities generally consist of planting, plant maintenance (watering, weeding, pruning etc), greenhouse maintenance (cleaning etc), landscaping and other related activities. Staff Qualifications:
Trained and holds current CPR/ First Aid certification
Trained and holds current Medication Assistance certification
Trained and holds current Mandt (behavior management) certification
Free of communicable disease in any form.
Cleared through criminal background investigation and drug testing.
Has demonstrated skills and abilities in supporting and training people with cognitive/physical disabilities.
Accreditation & Licensure:
Eggleston Services, Inc. Tanner Creek’s programs are CARF accredited in Organizational Employment Services and Personal and Social Support Services. It is also licensed through the Department of Mental Health, Mental Retardation and Substance Abuse Services.
